If you are short- or far-sighted then enter your dioptre values. For corneal irregularities you will also need your cylinder and axis values. The pupil distance value is important to maintain perfect vision. For varifocals you will also need to enter your addition values (shortened to “ADD” in the order form).
You will find this information on your eye prescription form. This document contains all the relevant details for your personal prescription. An important value, which is often not included in your eye prescription form is the “pupil distance“ (PD).
